Position: Care Manager

We are seeking a highly motivated and compassionate individual to join our team as a Care Manager. As a Care Manager, you will play a crucial role in overseeing the care and well-being of our clients. This is a full-time, permanent position with the potential for growth within our company.

Responsibilities

  • Develop and implement care plans for clients based on their individual needs and preferences
  • Coordinate and monitor the delivery of care services by our team of caregivers
  • Conduct regular assessments and evaluations of clients' physical, emotional, and mental well-being
  • Collaborate with healthcare professionals, family members, and other stakeholders to ensure the best possible care for clients
  • Maintain accurate and up-to-date records of clients' care plans and progress
  • Provide support and guidance to caregivers, including training and supervision
  • Advocate for clients' needs and rights, ensuring that they receive the highest quality of care
  • Monitor and manage clients' budgets and assist with financial planning, if needed
  • Stay up-to-date with industry trends and best practices in care management

Requirements

  • Bachelor's degree in social work, nursing, or a related field
  • Minimum of 2 years of experience in care management or a related field
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work independently and as part of a team
  • Compassionate and empathetic nature
  • Knowledge of healthcare systems and resources
  • Familiarity with medical terminology and conditions
  • Proficient in Microsoft Office and electronic medical record systems
  • Must pass a background check and drug test
