Rough Sleep Support Worker

We are currently supporting a Local Authority to recruit a Rough Sleep Support Worker on a temporary basis to Contribute to achieving the overarching aim of the Council’s Rough Sleeping Service to ensure rough sleeping is rare, brief and non-recurring through the provision of timely, effective and multi-disciplinary interventions.

Hourly rate: £34.75 (Umbrella)

Length of Contract: 3 months

Hours: 37 | Monday to Friday | 09:00 – 17:00

Location: Northampton

Key responsibilities:

Deliver proactive street outreach services, engaging rough sleepers and providing rapid support, accommodation pathways and crisis intervention

Build trusting relationships with rough sleepers, offering intensive support to access housing, healthcare, treatment services and wider support networks

Complete risk assessments and support plans, ensuring individuals receive tailored interventions and safeguarding support

Support individuals to develop independent living skills, sustain tenancies and move into long-term accommodation

Work closely with Housing Options, Adult Social Care and partner agencies to deliver personalised housing plans and positive outcomes

Coordinate and participate in multi-agency case conferences for individuals with complex needs, repeat homelessness or entrenched rough sleeping

Work collaboratively with hospitals, prisons, landlords and supported housing providers to prevent homelessness and rough sleeping

Act as an advocate and key point of contact for rough sleepers, ensuring access to appropriate services and support

Contribute to rough sleeper counts, SWEP provision and wider initiatives aimed at reducing rough sleeping across the area

Maintain accurate case records and ensure all interventions are recorded in line with policy, safeguarding requirements and service standards
