About this role…

We are looking for a Lead Worker to join the Hackney Street Outreach Rough Sleepers Team.

The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

• Conducting regular outreach shifts, sometimes with volunteers.
• Manage a caseload of complex clients alongside your colleagues, identifying the quickest and most appropriate route off the streets for them, and devising a plan to achieve this.
• Liaise with, signpost, and support client engagement with specialist agencies as appropriate e.g. drug treatment services, Community Mental Health Teams, NHS services, community groups etc.
• Identifying gaps in local provision and researching new pathways off the streets for a variety of rough sleepers, including those with no recourse to public funds.
• Work across a variety of databases to accurately record your work.
• Take a lead role within the team, working closely with support workers, volunteers and student placements around casework and resource provision for individuals accessing the service.
