Care Navigator (Primary Care Nurse / Nurse Practitioner)

Full-time | Hybrid (Remote + In-person)

We’re looking for an experienced Primary Care Nurse or Nurse Practitioner to join us as a Care Navigator — a pivotal role at the heart of our service.

The Role

This is not a traditional nursing role.

As a Care Navigator, you’ll be the first point of contact for patients, combining clinical expertise with triage, coordination, and relationship-building. You’ll guide patients through their healthcare journey, ensuring they feel supported and understood at every step.

You’ll work closely with GPs and senior clinicians, while helping shape how a modern, digitally enabled practice operates.

Key Responsibilities

Patient Care & Navigation

Act as primary contact, delivering a personalised patient experience

Triage queries, assess risk, and escalate appropriately

Coordinate appointments, referrals, tests, and follow-ups

Support patients navigating NHS and private pathways

Provide continuity, reassurance, and advocacy

Clinical Coordination

Work alongside clinicians to deliver safe, joined-up care

Escalate clinical concerns appropriately

Maintain accurate records in line with CQC and GDPR

Operational Development

Help design care pathways and service models

Engage with and improve digital/AI-enabled systems

Support workflow development in a growing startup

What We’re Looking For
