What you will be responsible for:
Report to the Clinical Site Team, attending 08:30 site huddle, to obtain an overview of site pressures, and names of any definite discharges that can be pulled to the Discharge Lounge (DCL)
Take accurate handovers- assessing the suitability of the patient for the DCL.
Access EDN, pharmacy and transport systems to ascertain any outstanding tasks for the patients safe discharge.
Excellent communication with patients, relatives and all internal and external staff- aiming to build relationships and promote the use of the DCL.
You'll have the following qualification/experience:
Statement of NMC Registration
Degree /Diploma in Nursing
A minimum of 6 months paid experience in an Acute hospital environment within the last 3 years.
