Job Description

Grosvenor Hart Homes (GHH) are looking for a Practitioner to join us and be based in Knowsley, Merseyside, as we expand our multi-disciplinary team.

Grosvenor Hart Homes is a new social enterprise on a mission to improve the life chances of children, young people and their families. We became a registered provider of social housing in June 2024. This is an additional role, is for 5 days a week and is a customer facing position.

Addressing three critical foundations, we provide high-quality affordable homes simultaneously paired with employment and wellbeing support programmes, tailored around the needs of individuals, and designed to empower them to overcome barriers, fulfill their potential and achieve better life outcomes.

The ideal candidate will have a professional qualification in social work, youth work or allied profession and must have previous experience of working with children, young people and their families as well as a sound knowledge of safeguarding procedures.

This is a unique opportunity to work with a small number of people in an impactful way. You will be somebody with the ability to form excellent relationships to develop and drive forward plans to help our tenants to achieve their ambitions. The role will involve working with children, young people and their families individually as well as promoting and facilitating community events such as life skills workshops, social events and wellbeing sessions.

Key responsibilities include:

Building trusted relationships with families and young people prior to, during and after their tenancy within Grosvenor Hart Homes.

Prior to moving in, work with each prospective tenant / household and the relevant statutory services to assess support needs and create a bespoke plan to meet those needs moving through to the next stage of their housing pathway, setting clear, measurable outcomes.

Discuss, agree, and then implement a move in plan to smoothly transition each tenant into their home.

Drive and oversee execution of each tenant’s support plan – removing barriers, helping to solve problems, keeping tenants motivated and focused on achieving the steps/ goals within their plan to its successful completion.

Offering practical and emotional support to individuals and families as needed, including in times of emergency or crisis.

Working with tenants to facilitate neighbourhood projects to further social outcomes, e.g., community gardening, arts, cooking activities.
