Were recruiting Support Workers to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.
This role supports people returning home from hospital to recover safely and regain independence doing with, not doing for.

What youll be doing
Supporting people at home after discharge with reablement-focused care
Assisting with personal care where needed, while encouraging independence
Supporting daily living skills (washing, dressing, meal prep, mobility, routines)
Following care plans and recording notes accurately
Reporting changes in condition, concerns, or risks promptly
