As a Care Coordinator, you will manage a caseload of service users, providing holistic assessments, care coordination, and recovery-focused interventions. You will support individuals with complex mental health needs, memory difficulties, reduced mobility, and social isolation, helping them maintain independence and improve their quality of life.

The role includes working within a multidisciplinary team and contributing to the ongoing development of the Memory Assessment Service, including the pre-diagnostic assessment pathway.

We are looking for caring, resilient, and motivated professionals with excellent communication skills and a commitment to delivering compassionate, person-centred care. As a Band 5 Care Coordinator, you will deliver high-quality community mental health care to older adults, managing a caseload and coordinating person-centred support in line with Trust policies and professional standards.

You Will

  • Undertake comprehensive mental health, risk and care needs assessments.
  • Develop, implement and review care plans, acting as Care Coordinator for a designated caseload. Work collaboratively with service users, carers, multidisciplinary teams and partner agencies.
  • Identify and manage risks, including deterioration in mental health, self-harm, suicide risk and safeguarding concerns. Coordinate referrals and access to appropriate services and interventions.
  • Monitor, administer and support compliance with prescribed medication in line with Trust policies and professional guidelines.
  • Maintain accurate, timely and comprehensive clinical records.
  • Promote recovery, independence, equality, diversity and inclusion in all aspects of care.
  • Work within the requirements of relevant legislation, including the Mental Health Act.

We are seeking motivated and compassionate clinicians with experience of working with people experiencing mental health difficulties.
