Job Description

We are seeking a compassionate and dedicated Complex Care Workers to provide specialised support to individuals with complex health needs in their own home on either a 1:1 or 2:1 basis. The successful candidate will deliver personalised care within clients' own homes, ensuring their comfort, safety, and wellbeing. This role offers an opportunity to make a meaningful difference in people's lives by delivering tailored care plans and fostering independence.

Responsibilities

Deliver personalised care in accordance with individual care plans, ensuring all needs are met with dignity and respect.

Assist clients with daily activities such as bathing, dressing, feeding, and mobility support.

Support clients with medication management and health monitoring as required.

Maintain accurate records of care provided and report any changes in client condition to relevant healthcare professionals.

Facilitate effective communication with clients, their families, and multidisciplinary teams to ensure seamless care delivery.

Drive clients to appointments or activities when necessary, ensuring safe transportation.

Promote independence whilst providing necessary assistance in a caring and empathetic manner.

Adhere to all health and safety regulations, including infection control procedures.

12 hour shifts working on either a 1:1 basis or 2:1 basis

Experience

Previous experience working within complex care is essential, such as mental health, brain injury, learning disabilities etc.

Knowledge of care plans and their implementation is advantageous.

Good IT skills for maintaining accurate documentation and communication records.

Strong communication skills in English, both verbal and written, to effectively interact with clients, families, and colleagues.
